  • Overview

    Position located at The Grimes Center

     

    The primary function of Advanced Nurse Practitioner/Physician's Assistant working for NEMG YNH Geriatric Services is to provide comprehensive care to adult patients in long & short term care at the Grimes Center. The APRN/PA work in collaboration with attending physicians. Each APRN works with the M.D. to provide outstanding and coordinated medical care. The APRN/PA also participates in education of students and fellows.  Exemplifies behaviors reflecting Service Excellence and support of the Strategic Dimensions by modeling the Core Success Factors as described in the Performance Review.

    Responsibilities

    • 1. Clinical Practice:
      • 1.1 Coordinates patient care plan with attending M.D. and nursing staff at Grimes. Communicates with other providers as needed.
    • 2. Education
      • 2.1 Identifies and provides information needs of team members, patients and families.
    • 3. Quality Improvement/Customer Satisfaction
      • 3.1 Participates in team based quality improvement and patient/customer satisfaction initiatives.
    • 4. Patient Safety
      • 4.1 Practices medicine in a manner to maximize patient safety.
    • 5. Operations
      • 5.1 Follows all NEMG YNH Geriatric Services policies and procedures.

    Qualifications

    EDUCATION

     

    APRN: Graduation from an accredited APRN program required. Licensed APRN by the State of CT. Holds a valid Connecticut medical License. Connecticut Controlled Substance Registration and Federal Drub Enforcement Agenecy (DEA) Certification.

     

    PA: Graduation from an accredited Physician Assistant program required. Certified a Physician Assistant by the National Commission on Certifications of Physician Assistants.  Holds a valid Connecticut medical License. Connecticut Controlled Substance Registration and Federal Drug Enforcement Agenecy (DEA) Certification.

     

    EXPERIENCE

     

    One (1) to three (3) years caring for patients preferred.

     

    SPECIAL SKILLS

     

    Excellent clinical, interpersonal and organizational skills. Ability to work independently with minimal supervision.

     

    PHYSICAL DEMAND

     

    MMR, Tdap and influenza vaccination are required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids.
