SLEEP MEDICINE

DAYS- OFFICE BASED POSITION - NEW LONDON, CT

The primary functions of the Nurse Practitioner will be to provide comprehensive care to the patients and consultation service to the patients direct care team under the supervision of the Physician and as directed by established protocols. In collaboration with the Health care team, the Nurse Practitioner will provide continuity of care to patients in outpatient setting to achieve optimal patient outcomes and patient satisfaction.

Responsibilities

  • - Record the history of present illness, past medical, family and social history 
    • Perform physical examinations 
    • Establish and follow plan of care in conjunction with MD 
    • Prescribe appropriate medications to a specialty pharmacy that will handle prior authorization and other necessary paperwork; Order refills as needed 
    • Patient and family teaching 
    • Order appropriate labs, tests and procedures and follow up with patient as needed 
    • Coordinate care and refer to other medical professionals as needed 
    • Respond to patient messages via clinic, telephone, and other patient encounters in the Epic system 
    • Review lab results in Epic, communicate and interpret results 
    • Collaborate with nursing staff to complete patient paperwork including medication pre-certifications for insurance companies, durable medical equipment requests, disability paperwork, and handicapped permit applications Sleep Medicine Responsibilities 
    • The APP will see sleep apnea patients and patients with a variety of other sleep disorders. 
    • The APP will be expected to manage devices used for treatment of sleep apnea such as CPAP machines. Prescribe PAP machines and evaluate downloaded data. 
    • Position will be responsible for coordinating care of sleep medicine patients with the CPAP coordinator and DME companies. 

Qualifications

 

  • Must be Board Certified as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • Must be licensed (or able to obtain license) as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Connecticut
  • Certification in Basic Life Support with AED required
  • Experience in pulmonary and/or sleep medicine is desirable
  • Must meet or exceed continuing education requirements for licensure

 

SPECIAL SKILLS

 

Excellent basic knowledge with clinical skills and judgment. Possess analytical skills to evaluate care needs, ability to gather relevant clinical date, perform through physical examinations, and possess excellent interpersonal skills, communication and leadership activities and exhibit good judgment and tact dealing with patients, families, and allied health professionals. Computer skills preferred.
